As HWF focuses on whole-person care, we wanted to unify the infrastructure that connects health and social care: enter UniteUs! This platform’s end-to-end solution establishes a new standard of care that identifies and predicts social care needs in communities.
Unite California is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us' shared technology platform, which enables them to send and receive electronic referrals, address the social needs of our clients, and create pathways to improved health, equity, and wellness. Our partnership’s goal is to leverage our vast network and resources to create meaningful outcomes in our amazing State of California!
